Petition For Change of Name of Adult

About this Form: In most states, a Petition for Change of Name of Adult should be filed with the county court in which the Petitioner (i.e. the individual seeking to change his or her personal name) resides. Any lawful and legal reason that the Petitioner considers a benefit is in most cases usually sufficient enough to satisfy most law courts. For instance, the Petitioner may wish to change their name because of social or religious reasons. However, the Petitioner may not change his or her name for purposes of fraud and/or if it violates the lawful rights of other individuals.
