Petition For Change of Name of Minor

About this Form: In most states, a Petition for Change of Name of Minor should be filed with the county court in which the minor resides. Any lawful and legal reason that the Petitioner(s) considers a benefit is in most cases is usually sufficient enough to satisfy most law courts. For instance, the Petitioner(s) may prefer a new name for the minor because of social or religious reasons. However, the Petitioner(s) may not change his or her name for purposes of fraud and/or if it violates the lawful rights of other individuals. Also, the Petitioner(s) are advised to make contact with the court clerk to find out the filing fees, court docket number, and/or other local law requirements.
